By submitting this intake form, you agree to the following terms:
Content Creation & Usage Rights
• You grant us permission to use all submitted materials (logos, images, brand information) solely for creating your personalized video advertisements
• You confirm you own or have legal rights to all submitted materials
• We retain the right to use created videos as portfolio examples unless specifically requested otherwise
Service Delivery
• Video production timelines will be communicated after form submission
• Revisions are limited to [X] rounds per video to ensure timely delivery
• Final videos will be delivered via your client portal for download
Social Media Posting (Pro Plan)
• You authorize us to post content to your connected social media accounts
• You maintain full ownership of your social media accounts and can revoke access at any time
• We are not responsible for platform algorithm changes or posting restrictions
Payment & Subscription
• Service begins upon successful payment through our Shopify store
• Subscription terms are as selected at time of purchase
• Cancellations must be requested [X] days before next billing cycle
Liability
• We are not responsible for social media platform policy violations
• You are responsible for ensuring all submitted content complies with applicable laws
• Our liability is limited to the amount paid for services
Data & Privacy
• Submitted information is used solely for service delivery
• We do not share your information with third parties
• You may request data deletion upon service termination
